Staying Home After Getting Home

After Patients are discharged from the hospital, the task of keeping up with home infusion medications and a new routine is daunting. On the other hand, Patients are looking forward to getting back home and in their familiar space. The health and wellness of our Patients is of upmost importance to us at Landmark. We provide clinical support along with each Patient's unique IV therapy to help our Patients not have to be readmitted to the hospital after discharging. At Landmark, our entire team of pharmacists, pharmacy technicians, customer service professionals, nurses, and pharmacy staff are available to answer any questions, help with any issues, and coordinate with home health agencies to provide support from start to finish. Taking steps to prevent an avoidable readmission has the potential to improve your quality of life and save you money. Once you are discharged from the hospital, it is important to follow these 7 tips that can help you stay home once you get home.


  1. Follow the directions that your pharmacist has laid out for you including the frequency, time of day, and proper dosage instructions to ensure you are infusing your medication in the way it was prescribed.

  2. Attend all Doctors' appointments with your physician and follow your plan of care that your Physician and any collaborating clinicians have outlined for you.

  3. If you run into any problems with your home infusion therapy, call your prescribing physician or pharmacist. Our pharmacist on-call can be reached 24/7 at 866-388-3883. Please remember that the Landmark Pharmacy team will be calling you weekly to discuss your medication and any refills if needed.

  4. Communicate with home health nurses about any ongoing symptoms you may have so they can best help you manage your therapy.

  5. Make sure your home is easy to navigate and move around in and everything you need for the first few days is within a short distance of the room you will be staying in.

  6. Be aware of any medication or food interactions that may cause a negative reaction with your current home infusion therapy and eat a balanced diet.

  7. Maintain a healthy, clean environment in your home. It is important to prevent infections when dealing with IV medications and equipment so that you don't have additional complications with your therapy. Use gloves and alcohol pads to lessen the spread of germs at the IV site.

  • Bleeding that will not stop
